Two ways to buy branded Wegovy as a self-paying patient in the United States sit on the same supply chain and cost very different amounts. NovoCare Pharmacy is Novo Nordisk's own self-pay channel, selling the manufacturer's own pens at a published price with no platform fee. Ro is a telehealth platform that bills a recurring membership and then bills the medicine separately, and Ro is one of the partners Novo Nordisk named when it set those self-pay prices. On the figures we could read on 10 October 2026, a modelled twelve months of Wegovy injection came to about $3,888 direct from NovoCare Pharmacy and about $5,522 to $5,566 through Ro on a month-to-month membership. The medicine price is the same number in both columns. The gap is the membership and almost nothing else. This article shows the arithmetic, names the source and date behind every figure, and is explicit about the two places where our evidence is weaker than we would like.
DoseTruth is a price-comparison publisher: it is not a pharmacy, not a telehealth provider, and it does not prescribe or dispense any medication. This article is general information about published prices and published regulatory documents, it is not medical advice, and every decision about a medicine, a dose or a titration schedule belongs with a licensed clinician. Every price below carries the date it was checked and the source it came from. Self-pay prices in this market have moved several times in the past year, so confirm the live figure on the day you pay.
What does a year of Wegovy cost through NovoCare directly?
On the NovoCare Pharmacy Wegovy page we read on 10 October 2026, the self-pay price is $199 per month for the first two monthly fills at 0.25 mg and 0.5 mg for new patients, then $349 per month for 0.25 mg, 0.5 mg, 1 mg, 1.7 mg or 2.4 mg. Wegovy HD at 7.2 mg is $399 per month. The page defines one month as one box of four pens, states that shipping and delivery are free, and states that the introductory pricing runs through 31 December 2026. It also states that self-pay prescriptions will not count toward any deductible, which is the trade-off for the low cash price.
Working the published unit prices through twelve fills gives $199 plus $199 plus ten fills at $349, which is $3,888. That is our calculation from the published prices, not a total quoted by Novo Nordisk, and it assumes a person stays on the injection for twelve consecutive months and captures the two-fill introductory offer. A patient who is not new to self-pay, or who starts after the introductory window closes, pays twelve fills at $349, which is $4,188. We model the oral and injection routes separately because the price ladders differ, and we have written about the oral ladder in the oral Wegovy 4 mg price rise from $149 to $199.
| Product and dose | Self-pay price per month | Condition stated on the page | Source tier |
|---|---|---|---|
| Injection 0.25 mg and 0.5 mg, new patients | $199 | First 2 monthly fills, through 31 December 2026 | Manufacturer's own pharmacy page |
| Injection 0.25 mg to 2.4 mg, standard | $349 | Self-pay only, no insurance, no deductible credit | Manufacturer's own pharmacy page |
| Injection 7.2 mg, Wegovy HD | $399 | Self-pay only | Manufacturer's own pharmacy page |
| Pill 1.5 mg | $149 | One month equals one bottle of 30 tablets | Manufacturer's own pharmacy page |
| Pill 4 mg | $199 | Self-pay only | Manufacturer's own pharmacy page |
| Pill 9 mg and 25 mg | $299 | Self-pay only | Manufacturer's own pharmacy page |
One structural point separates this route from the Lilly route we audited in LillyDirect versus Hims, the total cost of one year. We found no annual fill cap stated on either NovoCare page we read on 10 October 2026, and no 45-day refill window of the kind that governs the Lilly self-pay price. We are stating the absence of a condition on the pages we read, which is weaker than a positive statement that no cap exists anywhere in the programme terms. Treat it as read and not as proven.
What does a year of Wegovy cost through Ro?
Ro bills two separate things, and this is the part that defeats most mental arithmetic. The Ro Body membership is one recurring charge and the medicine is another, on its own schedule. A Fortune review of Ro Body dated 9 October 2026 states the membership is $39 for the first month and $145 per month after that, plus the cost of medication, and states that medications are not included in the membership fee. A Healthline review updated 26 August 2026 states $45 for the first month and then $149 per month in one place and $145 in another, which is an internal contradiction in a tier three source. Commercial comparison pages we opened put it at $39 then $149, with prepay tiers of $99 per month on a three-month plan, $89 on six months and $74 on a twelve-month plan.
We attempted to read the Ro pricing page itself on 10 October 2026 and it returned HTTP 403 on every attempt, so we could not confirm the membership fee against the company's own published page. The exact Ro membership fee is the weakest number in this article and we model it as a range rather than a point. The honest statement is that the monthly membership sits between $145 and $149, with a first month discounted to $39 or $45, and that the prepay tiers rest on commercial sources only.
| Billing plan | Stated membership price | Source and date | Source tier |
|---|---|---|---|
| Month to month, first month | $39, or $45 in one source | Fortune, 9 October 2026, and Healthline, updated 26 August 2026 | Reputable secondary, two sources disagree |
| Month to month, ongoing | $145 to $149 | Fortune says $145, Healthline says both $145 and $149 | Reputable secondary, internally inconsistent |
| Three-month prepay | $99 per month | Commercial comparison page, verified 7 May 2026 | Not primary, commercial source only |
| Six-month prepay | $89 per month | Commercial comparison page, verified 7 May 2026 | Not primary, commercial source only |
| Twelve-month prepay | $74 per month, $888 for the year | Fortune, 9 October 2026, and commercial pages | Reputable secondary plus commercial |
| Company's own pricing page | Could not read | ro.co returned HTTP 403 on 10 October 2026 | Unavailable |
On the medicine side the picture is clearer, and it is the single most useful fact in this comparison. Novo Nordisk's own press release of 17 November 2025 sets the $199 introductory price and the $349 standard self-pay price, and names the channels those prices run through: NovoCare Pharmacy, more than 70,000 retail pharmacy locations, and telehealth partners including Costco, GoodRx, WeightWatchers, Ro, LifeMD and eMed. Ro does not sell a cheaper Wegovy, it sells the same manufacturer self-pay price with a membership stacked on top. A commercial comparison page verified 7 May 2026 lists Ro's Wegovy injection at $199 for the intro doses and $349 ongoing, which matches the manufacturer price exactly, and that match is what we would expect from a pass-through partner.
What do the two routes total over twelve months?
Holding the medicine column identical and changing only the membership gives the table below. All four totals are our arithmetic on published unit prices and none is a figure quoted by either company.
| Route | Medicine for 12 fills | Platform fee for 12 months | Twelve-month total | Premium over direct |
|---|---|---|---|---|
| NovoCare Pharmacy, intro captured | $3,888 | $0 | $3,888 | Baseline |
| NovoCare Pharmacy, no intro | $4,188 | $0 | $4,188 | $300 |
| Ro, month-to-month at $145 | $3,888 | $1,634 | $5,522 | $1,634, or 42 per cent |
| Ro, month-to-month at $149 | $3,888 | $1,678 | $5,566 | $1,678, or 43 per cent |
| Ro, twelve-month prepay at $74 | $3,888 | $888 | $4,776 | $888, or 23 per cent |
Three things fall out of that table. First, on a month-to-month membership the platform fee is about 30 per cent of what the patient pays across the year, for a medicine whose price the platform does not change. Second, the prepay plan cuts the premium from roughly $1,650 to $888, which is the single largest saving available on the Ro route and is invisible to anyone comparing headline monthly figures. Third, the $300 gap between the two NovoCare rows is smaller than most readers expect, because the introductory offer is only two fills deep. The introductory price is worth $300 across a year and the membership decision is worth about $1,650, so the membership is roughly five times the more consequential choice. We made the same point about front-loaded offers in what $25 for the first month actually costs over a year.
We should also flag what we did not model. We could not verify whether the $39 or $45 first-month promotion stacks with the twelve-month prepay plan, so the $888 figure assumes it does not. We also saw a commercial page listing a discounted Wegovy price of $249 per month on a twelve-month Ro subscription, and we could not corroborate that anywhere else, so it is absent from the table. If that price is real it would change the ranking materially, and we would rather leave the gap visible than fill it with one unverified number.
Which route needs a new prescription?
This is the question that changes the real entry cost, and the two routes answer it differently. The NovoCare Pharmacy page we read on 10 October 2026 states that Wegovy requires a prescription, that the patient should ask their own health care provider and have them send the prescription to NovoCare Pharmacy, and that an existing prescription can be transferred by calling 1-833-949-5527. In other words the route accepts a prescription a patient already holds, from a clinician they already see.
Ro works the other way. The Fortune review of 9 October 2026 states that the intake questionnaire is completed without a live provider, after which a Ro-affiliated provider reviews the information. The clinical decision sits inside the platform, and the membership is the gate to that provider relationship. We found no evidence that Ro will take an outside prescription and dispense against it without a membership, and we also found no explicit statement that it refuses to, so we are reporting an absence rather than a prohibition.
| Requirement | NovoCare Pharmacy | Ro |
|---|---|---|
| Prescription needed | Yes, stated on the page | Yes, written by a Ro-affiliated provider |
| Accepts an existing prescription | Yes, your provider sends it, or transfer by phone | Not evidenced either way |
| Clinical visit inside the service | No, the prescriber is your own clinician | Yes, intake reviewed by a Ro-affiliated provider |
| Recurring platform fee | None found on the pages read | $145 to $149 per month, or $74 prepaid annually |
| Shipping | Free, stated on the page | Not verified, company page unreadable |
| Counts toward a deductible | No, stated on the page | Not verified |
The practical reading is narrow and worth stating plainly. A person who already has a prescribing relationship and a current Wegovy prescription is paying the Ro membership for care they already have access to elsewhere. A person with no prescriber, no appointment and no obvious route to one is buying something real with that fee, and whether $1,650 a year is a fair price for it is a judgement about their own circumstances, not an arithmetic error. Our reading of the pricing in advertised price versus maintenance dose applies here too, because the advertised entry number on both routes is the shallow end of the ladder.
How strong is the evidence behind each claim here?
Weakest first, because that is the order a sceptical reader should audit us in.
| Claim | Evidence type | Strength |
|---|---|---|
| Ro Wegovy at $199 intro and $349 ongoing | One commercial comparison page, verified 7 May 2026, consistent with the manufacturer press release channel list | Weak on its own, moderate as corroboration |
| Ro prepay tiers at $99, $89 and $74 | Commercial pages, with $74 also in Fortune | Weak, except the $74 tier which is moderate |
| Ro ongoing membership of $145 to $149 | Fortune 9 October 2026 says $145, Healthline says both, company page unreadable | Moderate, and internally contradictory |
| Ro will not dispense against an outside prescription | No source found either way | Not claimed, absence of evidence only |
| No annual fill cap on the NovoCare self-pay route | Absence of a stated cap on two pages read | Weak, absence is not proof |
| NovoCare self-pay prices in Table 1 | Manufacturer's own pharmacy page, read directly | Strong |
| Existing prescriptions can be transferred to NovoCare | Manufacturer's own pharmacy page, with a phone number | Strong |
| Ro is a named Novo self-pay channel partner | Novo Nordisk press release, 17 November 2025 | Strong, corporate primary document |
| Medicare negotiated price of $274 from 2027 | CMS announcement of 25 November 2025, reported by trade press | Strong on the figure, irrelevant to self-pay buyers |
| Twelve-month totals in Table 3 | Our arithmetic on verified unit prices | Strong arithmetic, assumption-dependent |
Two of those rows deserve more than a label. We could not read Ro's own pricing page, which means the central fee in a cost comparison about fees rests on secondary reporting that disagrees with itself by $4 a month. And we could not cleanly re-verify NovoCare's Ozempic self-pay prices today, because the page we opened returned a price block that mixed injection and tablet figures, so we make no Ozempic price claim here at all and point instead to the 17 November 2025 press release, which set Ozempic at $349 standard with the 2 mg strength held at $499.
What changes on 1 January 2027?
Both columns move, and not by the same amount. Novo Nordisk announced on 24 February 2026 that from 1 January 2027 Ozempic, Wegovy and Rybelsus will carry a single monthly list price of $675, roughly halving the Wegovy list price from the $1,349 reported in March 2025. Separately, CMS announced negotiated Medicare prices on 25 November 2025, setting semaglutide at $274 for a thirty-day supply effective January 2027, covering Ozempic, Rybelsus and Wegovy. Neither change is a self-pay price. The list price governs what people on high-deductible plans are exposed to, and the negotiated price governs Medicare Part D, so a cash buyer at $349 today is below both numbers already.
The date that actually matters to the arithmetic above is 31 December 2026, when the NovoCare introductory offer lapses according to the page we read. A patient who starts the injection in November 2026 captures both introductory fills, and a patient who starts in January 2027 captures neither, a $300 swing on an identical year of medicine. We track that cluster of expiries in the GLP-1 price routes 31 December 2026 reset. The membership side has no equivalent deadline that we could verify, which is itself worth noting, because a recurring fee with no expiry is the one line in Table 3 that does not improve with time.
Frequently asked questions
Is the Wegovy sold through Ro the same product as the Wegovy from NovoCare Pharmacy?
Both routes describe branded Wegovy from Novo Nordisk, and Novo Nordisk's press release of 17 November 2025 names Ro as one of the telehealth channels carrying its self-pay price. We have no evidence of any difference in the product itself. That is a statement about supply and labelling, not about whether the medicine is appropriate for any individual, which is a clinical question for a licensed clinician.
Can I keep my own doctor and still get the $349 price?
The NovoCare Pharmacy page we read on 10 October 2026 states that you ask your own health care provider for a prescription and have them send it to NovoCare Pharmacy, and that an existing prescription can be transferred by phone. On that page's own terms, yes. Whether your clinician will write it is entirely their decision.
Does the Ro membership ever pay for itself?
Not through the medicine price, on the evidence we found, because the medicine price appears to be the same manufacturer self-pay figure on both routes. It could pay for itself through the clinical access and insurance navigation it bundles, and Ro states that it accepts insurance for medication, which could matter a great deal to a person with partial coverage. We could not verify any insurance outcome, so we are not putting a number on it.
Why is a year eleven payments of membership and not twelve?
Because the first month is discounted, so twelve months of membership is one promotional payment plus eleven standard ones. This is the same billing shape that makes quarterly and annual plans so hard to compare at a glance, which we unpacked in Noom bills by the quarter, not the month.
Is a discount card route cheaper than either of these?
Not in the comparison we ran previously, where the manufacturer's own channel beat the card route on the same pens. See why GoodRx costs more than buying Wegovy direct for that arithmetic and its limits.
How much of this article could you not verify?
The Ro membership fee to within $4 a month, the Ro prepay tiers beyond the annual one, whether the first-month promotion stacks with prepay, Ro's shipping cost, Ro's deductible treatment, whether Ro accepts an outside prescription, and a reported $249 Wegovy price on a twelve-month Ro plan. Every one of those gaps sits on the Ro side of the table, for the simple reason that the company's pricing page would not open for us and the manufacturer's would.
